Considering that microbial resistance to antibiotics is becoming an increasingly widespread problem, burn management, which usually includes the use of topical antimicrobial dressings, is still facing difficulties regarding their efficiency to ensure rapid healing. In this context, the main objective of this research is to include new oxytetracycline derivatives in polymeric-film-type dressings for the treatment of wounds caused by experimentally induced burns in rats. The structural and physico-chemical properties of synthesized oxytetracycline derivatives and the corresponding membranes were analyzed by FT-IR and MS spectroscopy, swelling ability and biodegradation capacity. In vitro antimicrobial activity using Gram-positive and Gram-negative bacterial strains and pathogenic yeasts, along with an in vivo study of a burn wound model induced in Wistar rats, was also analyzed. The newly obtained polymeric films, namely chitosan-oxytetracycline derivative membranes, showed good antimicrobial activity noticed in the tested strains, a membrane swelling ratio (MSR) of up to 1578% in acidic conditions and a biodegradation rate of up to 15.7% on day 7 of testing, which are important required characteristics for the tissue regeneration process, after the production of a burn. The in vivo study proved that chitosan-derived oxytetracycline membranes showed also improved healing effects which contributes to supporting the idea of using them for the treatment of wounds caused by burns.

Background Though various advancements came into the field of surgery to do the atraumatic procedure, post-operative pain, and swelling are unavoidable complications. Hence, various medicaments are packed in the extracted third molar sockets to prevent these post-operative complications. Aim The study aimed to evaluate the efficacy of oxytetracycline hydrocortisone-soaked gauze in reducing post-operative pain and swelling compared to conventional surgical procedures without any packing in patients undergoing surgical extraction of the impacted mandibular third molars. Materials and methods The study was conducted in the Department of Oral and Maxillofacial Surgery at Saveetha Dental College and Hospitals, Chennai. In this study, 50 patients were randomly included in two groups of 25 participants each. In group A, oxytetracycline hydrocortisone-soaked gauze was placed, and in group B, conventional closure was done without any pack after surgical removal of impacted mandibular third molars. Post-operative pain was assessed on days one, three, and five using a 10-point visual analog scale. Post-operative swelling was assessed on the third and seventh days using a four-point swelling measurement. Data analysis was done using SPSS (IBM Corp. Armonk, NY). A p-value less than 0.05 was considered statistically significant. Independent sample t-test was done to compare the outcomes between the two groups. Results The results demonstrated that group A (Oxytetracycline Hydrocortisone-soaked gauze) showed superior pain reduction compared to group B (conventional closure) at all post-operative intervals (P=0.001). Moreover, group A exhibited reduced swelling, resulting in higher patient satisfaction levels compared to group B on the third post-operative day (P=0.001). Conclusion It can be concluded from the study that there was a significant reduction in post-operative pain and swelling with the use of oxytetracycline hydrocortisone-soaked gauze, as it acts like a local drug delivery system in patients undergoing impacted mandibular third molar surgeries.

PurposeRefractory periorbital dermatitis has a chronic course with exacerbations leading to discomfort and cosmetic issues, yet characterization of treatment options is limited.AimsThe objective was to present comprehensive demographic data and medical management of a series of patients with refractory periorbital dermatitis.Settings and designRetrospective review identified patients treated at a single institution from January 2010 to August 2020.MethodsDescriptive analyses were performed. Demographic data and treatment history were reviewed and data including medication, use, date of use and discontinued use, reason for discontinuation (if applicable), refractory status, formulation, concentration, and dose frequency were extracted.Statistical analysis usedDescriptive analyses.ResultsForty-five patients were included. The average age at first diagnosis was 60.3 years (sd 14.9). 82.2% were women and 84.4% identified as Caucasian. Triamcinolone cream was most frequently used followed by tobramycin-dexamethasone, tacrolimus, and neomycin-polymyxin-dexamethasone. Less than 30% of patients on triamcinolone were refractory. 13.3% of patients used topical hydrocortisone, with over 80% of these patients experiencing refractory episodes of persistent irritation and erythema. Most patients were refractory during initial use or the first recurrence of periorbital dermatitis flare.ConclusionsBy better characterizing the diverse treatment regimens in a unique subset of refractory patients, we hope to better inform potential courses of medical management for periorbital dermatitis.

Abstract Background : To describe the efficacy of clinician-administered hydrocortisone-oxytetracycline ear packing over three sessions in resolving uncomplicated otitis externa (OE). Case Presentation : A 34-year-old male with acute OE presented with severe pain, canal edema, and moderate discharge. Previous self-administered topical therapy had failed. Treatment involved three sessions of hydrocortisone-oxytetracycline-soaked ear packing over six days. Pain reduced by 40% at 48 hours and resolved completely by day 6. Edema and discharge progressively resolved, with full clinical recovery by day 6. No adverse events occurred. Conclusion : Structured ear packing offers a faster, safer alternative to conventional self-administered drops, ensuring rapid recovery and high patient comfort.

AbstractAimOur previous study proposed a technique of inserting gauze coated with oxytetracycline–hydrocortisone ointment into the extraction socket that suppressed the occurrence of dry socket and post‐extraction pain compared with the gauze‐free group. In this study, we estimated the incidence of dry socket and post‐extraction infection in patients with ointment gauze insertion and identified predictors of post‐extraction pain and change in maximum opening volume.MethodsThis retrospective study was carried out on patients who underwent lower third molar extraction in the Department of Oral Surgery at a general hospital in Japan.ResultsThe incidence of dry socket and infection after tooth extraction was 1.05% and 0.35%. The mean visual analog scale (VAS) on the operative day was 4.15; the VAS for postoperative day (POD)1 and POD7 were 2.28 and 0.55, respectively. The percentage change in maximum mouth opening on POD1 was −9.88%, and that on POD7 was −1.51%. Multivariable analysis indicated that age and Winter position classification were predictors of POD1 VAS, and female sex, smoking, and Winter position classification were predictors of POD7 VAS. Winter position classification and preoperative maximum mouth opening, respectively, were identified as predictors of maximum mouth opening percent change on POD1 and POD7.ConclusionsThe insertion of gauze coated with oxytetracycline–hydrocortisone ointment into the extraction socket of the mandibular third molar showed a low incidence of dry socket and post‐extraction infection. Moreover, the Winter position classification was suggested to be an independent predictor of postoperative pain and the maximum mouth opening percent change.
