Arnica L. genus (Asteraceae) comprises perennial herbs native to the temperate and boreal parts of the northern hemisphere. Arnica montana is the main species. It shows different biological activities, such as antioxidant, anti-inflammatory, antibacterial, antifungal, and antitumor effects. The Arnica formulations are mainly used for pain management. This systematic review is aimed at summarizing the studies focusing on the use of Arnica products on pain and inflammatory signs due to traumatic injuries related to sport and surgical interventions as well as to arthritis and other inflammatory conditions. Both phytotherapeutic and homeopathic formulations are taken into account. This paper only includes manuscripts published in mainstream journals. A literature search from Scopus, Web of Science, and PubMed databases has been carried out using a combination of the keywords "Arnica", "trauma", "sport", "injury", "injuries", and "pain". According to the search strategy and inclusion criteria for this study, 42 eligible papers, focusing on both Arnica alone and formulations containing a mixture of plant extracts, have been finally selected. This review critically discusses the in vitro, in vivo, and clinical studies dealing with Arnica products, reporting both positive and negative outcomes, thus providing perspectives for future research on the plant pharmacological potential.

Objectives: In Part 1 of the review of physicochemical research performed on homeopathic preparations the authors identified relevant publications of sufficient reporting quality for further in-depth analysis. In this article, the authors analyze these publications to identify any empirical evidence for specific physicochemical properties of homeopathic preparations and to identify most promising experimental techniques for future studies. Methods: After an update of the literature search up to 2018, the authors analyzed all publications in terms of individual experiments. They extracted information regarding methodological criteria such as blinding, randomization, statistics, controls, sample preparation, and replications, as well as regarding experimental design and measurement methods applied. Scores were developed to identify experimental techniques with most reliable outcomes. Results: The publications analyzed described 203 experiments. Less than 25% used blinding and/or randomization, and about one third used adequate controls to identify specific effects of homeopathic preparations. The most promising techniques used so far are nuclear magnetic resonance (NMR) relaxation, optical spectroscopy, and electrical impedance measurements. In these three areas, several sets of replicated high-quality experiments provide evidence for specific physicochemical properties of homeopathic preparations. Conclusions: The authors uncovered a number of promising experimental techniques that warrant replication to assess the reported physicochemical properties of homeopathic preparations compared with controls. They further discuss a range of experimental aspects that highlight the many factors that need to be taken into consideration when performing basic research into homeopathic potentization. For future experiments, the authors generally recommend using succussed (vigorously shaken) controls, or comparing different homeopathic preparations with each other to reliably identify any specific physicochemical properties.

Based on the widespread use of homeopathy in dairy farm practice when treating mastitis, a blind randomised controlled trial (RCT) was conducted to assess the effectiveness of homeopathic treatment of clinical mastitis on four dairy farms. The study considered specific guidelines for RCTs as well as the basic principles of individualised homeopathy and involved 180 lactating dairy cows. Evaluation of cure rates was based on clinical investigation of the udder and on laboratory analysis of milk samples. In culture-positive cases, the antibiotic treatment provided suboptimal bacteriological cures (60-81 per cent) but was more effective than individualised homeopathy (33-43 per cent) whose effects appeared little different to those of placebos (45-47 per cent) (P≤0.05). On the cytological cure level, all three treatment methods were similarly ineffective: antibiotic being 2-21 per cent, individualised homeopathy 0-8 per cent and placebo 3-13 per cent (P≤0.05; P=0.13). Antibiotics, individualised homeopathy and placebo had similar effects on bacteriological and cytological cure in cases of culture-negative milk samples (P>0.4) and Escherichia coli infections (P=1.0). The study results implied that the effectiveness of individualised homeopathy does not go beyond a placebo effect and successful treatment is highly dependent on the specific mastitis pathogen. Thus, antimicrobial or alternative remedies used should be based on the bacterial culture of the milk sample.Trial registration numberNTP-ID: 00008011-1-9, Pre-results.

BackgroundThe effectiveness of injection therapy for low-back pain is still debatable. We compared the efficacy of local injections of the homeopathic preparation Disci/Rhus toxicodendron compositum (verum) with placebo injections and with no treatment in patients with chronic low back pain.Methodology/principal findingsIn a randomized controlled partly double blind multicenter trial patients with chronic low back pain from 9 German outpatient clinics were enrolled and randomly allocated in a 1∶1∶1 ratio to receive subcutaneous injections (verum or placebo) into painful sites on the lower back over 12 treatment sessions within eight weeks, or no treatment (rescue pain medication with paracetamol or NSAIDs). All trial personnel and participants were masked to treatment allocation. The primary outcome measure was the average pain intensity over the last seven days on a visual analogue scale (0-100 mm, 0 = no pain, 100 = worst imaginable pain) after eight weeks. Follow-up was 26 weeks. Primary analysis was by intention to treat. Between August 2007 and June 2008, 150 patients were randomly allocated to three groups (51 verum, 48 placebo and 51 no treatment). The mean baseline-adjusted low back pain intensity at week eight was: verum group 37.0 mm (97.5% CI 25.3;48.8), no treatment group 53.0 (41.8;64.2), and placebo group 41.8 (30.1;53.6). The verum was significantly superior to no treatment (P = 0.001), but not to placebo (P = 0.350). No significant side effects were reported.Conclusions/significanceThe homeopathic preparation was not superior to placebo. Compared to no treatment injections resulted in significant and clinical relevant chronic back pain relief.Trial registrationClinicalTrials.gov NCT00567736.

BackgroundIn spite of recent advances in post-operative pain relief, pain following orthopedic surgery remains an ongoing challenge for clinicians. We examined whether a well known and frequently prescribed homeopathic preparation could mitigate post-operative pain.MethodWe performed a randomized, double blind, placebo-controlled trial to evaluate the efficacy of the homeopathic preparation Traumeel S in minimizing post-operative pain and analgesic consumption following surgical correction of hallux valgus. Eighty consecutive patients were randomized to receive either Traumeel tablets or an indistinguishable placebo, and took primary and rescue oral analgesics as needed. Maximum numerical pain scores at rest and consumption of oral analgesics were recorded on day of surgery and for 13 days following surgery.ResultsTraumeel was not found superior to placebo in minimizing pain or analgesic consumption over the 14 days of the trial, however a transient reduction in the daily maximum post-operative pain score favoring the Traumeel arm was observed on the day of surgery, a finding supported by a treatment-time interaction test (p = 0.04).ConclusionsTraumeel was not superior to placebo in minimizing pain or analgesic consumption over the 14 days of the trial. A transient reduction in the daily maximum post-operative pain score on the day of surgery is of questionable clinical importance.Trial registrationThis study was registered at ClinicalTrials.gov. # NCT00279513.

Complementary and alternative medicine (CAM) uses a holistic approach that finds natural solutions that help the immune system to fight off infection and diseases. In conventional medicine, the diseases are treated as a series of symptoms developed and not its actual cause or etiology, but CAM commonly targets the exact cause of the disorder thereby stimulating the body's healing process. This is based on an integrative literature review of methods and techniques used as complementary and alternative approaches for oral health care. A comprehensive electronic database search was conducted in PubMed, CINAHL, MEDLINE, EMBASE, Google, Google Scholar, and SCOPUS. Medicinal plants such as Medicago Sativa, Aloe Barbadensis Miller (Aloe Vera), and Trifolium Pratense (Red Clover) have excellent applications in treating gum disorders, prevent tooth decay, and have demonstrated good antifungal activity in the oral cavity. Homeopathic medicines such as Belladonna, Antimonium crudum, and Chamomilla have useful applications in relieving toothache. In Chinese medicine, various acupressure points (Acupuncture) have been used to relieve pain related to tooth, head-and-neck region, sinusitis, etc. Dental professionals can utilize these treatment modalities in their practice along with other conventional procedures as an integrative treatment approach to achieve better outcomes.

ObjectivesIn the absence of a definitive cure, herbal medications are gaining increasing popularity in the general public for treatment of coronavirus disease 2019 (COVID-19). Although many herbal preparations are safe and can be used without complication, serious toxicities do occur. This article focuses on the major characteristics and toxicities of herbal preparations that have been proposed as treatments for COVID-19.MethodsA review was performed focusing on herbal preparations that have gained popularity as potential treatments for COVID-19. Some of these preparations have been directly recommended by government agencies, whereas others have gained popularity through various other news sources.ResultsThe herbal preparations covered in this paper include the cardiac glycoside oleandrin, plants of the Datura genus, and herbs commonly used in traditional Chinese Medicine including plants of the Aconitum genus, bitter apricot seeds, ephedra, and licorice root.ConclusionAlthough herbal preparations have been reported to aid in the treatment of COVID-19 with success, few clinical trials have been performed to evaluate their efficacy and instead rely mainly on in vitro studies and anecdotal reports. Furthermore, many of the herbal preparations suggested carry significant toxicities, and frontline healthcare workers should be aware of the common symptoms and toxidromes that result from these poisonings.

Chitosan (CTS) is recognized for enhancing a plant’s resilience to various environmental stresses, such as salinity and drought. Moreover, salicylic acid (SA) is acknowledged as a growth regulator involved in addressing metal toxicity. However, the effectiveness of both compounds in mitigating Cr-induced stress has remained relatively unexplored, especially in the case of Aconitum napellus, a medicinally and floricultural important plant. Therefore, the primary objective of this study was to investigate the potential of CTS and SA in alleviating chromium (Cr)-induced stress in A. napellus. To address these research questions, we conducted a controlled experiment using potted plants to evaluate the individual and combined impacts of CTS and SA on plants exposed to Cr stress. Foliar application of CTS (0.4 g/L) or SA (0.25 mmol/L) led to significant improvements in the growth, chlorophyll content, fluorescence, and photosynthetic traits of A. napellus plants under Cr stress. The most notable effects were observed with the combined application of CTS and SA, resulting in increases in various morphological parameters, such as shoot length (2.89% and 7.02%) and root length (27.75% and 3.36%) under the Cr 1 and Cr 2 treatments, respectively. Additionally, several physiological parameters, such as chlorophyll a (762.5% and 145.56%), chlorophyll b (762.5% and 145.56%), carotenoid (17.03% and 28.57%), and anthocyanin (112.01% and 47.96%) contents, were notably improved under the Cr 1 and Cr 2 treatments, respectively. Moreover, the combined treatment of CTS and SA improved the fluorescence parameters while decreasing the levels of enzymatic antioxidants such as catalase (27.59% and 43.79%, respectively). The application also notably increased osmoprotectant parameters, such as the total protein content (54.11% and 20.07%) and the total soluble sugar content (78.17% and 49.82%) in the leaves of A. napellus in the Cr 1 and 2 treatments, respectively. In summary, these results strongly suggest that the simultaneous use of exogenous CTS and SA is an effective strategy for alleviating the detrimental effects of Cr stress on A. napellus. This integrated approach opens promising avenues for further exploration and potential implementation within agricultural production systems. This explains the interactive role of chitosan and salicylic acid in alleviating chromium stress in Aconitum napellus.
